ALERTNOW Information Form for Students
NOTE: This system does NOT replace all of the other current means of communicating a school cancellation - television and radio broadcasts, the Lincoln homepage, and the school answering system.
Those notifications will all continue to be operative. This is an addition to those systems already in place.
Please fill out one form for each child enrolled at Lincoln School.
In a NON-emergency situation (for example if school is cancelled because of snow or hurricane) the system would call the first two numbers below (Phone 1 and 2). That would not be an emergency situation, so you will receive a non-emergency notification call at that one telephone number.
Please keep that in mind when you are choosing the numbera to put in those spaces.
In an emergency situation (for example if we needed to have an early dismissal of all students because of a sudden unexpected severe change in the weather, problem at the school, or a national emergency) all five numbers (Phones 1-5) will be called. You might like to include the numbers of trusted relatives (grandparents, aunts, uncles) or friends who live near the school with whom you would feel comfortable entrusting your child should you be unreachable.
Do NOT include extensions with work numbers. This system is automated and will not dial extensions.
Please enter seven-digit phone numbers (area code + number) in the following format: xxx-xxx-xxxx. Do not include a preceding 1- or enclose the area code in parentheses.
Email messages follow the same guidelines as telephones above.

Please fill in this form for each of your children. The notification system does a cross-reference check of numbers in the database, so you should not receive multiple multiple phone calls even though you have multiple children.
