Required Health Information
Lincoln partners with Magnus Health to collect and store student’s health and medical information for the upcoming school year. This online database brings with it many benefits, including less paperwork for parents to complete each year, and up to date information that is accessible to parents and the Lincoln school nurses. Magnus Health allows families to to digitally submit/sign the forms online, and provides continuous access to their child’s health record as well as the ability to make updates as necessary. In addition, the Magnus system manages field trip permission forms, and other school-related forms.
Timely submission of health information and other forms is vital so that preparations can be made prior to the first day of school. Please plan to complete Magnus forms for your child(ren) by 8/1/24. For new families, the process should take approximately 20 minutes per child; less time is required for returning Lincoln students as information from last year will be pre-populated for you to update and/or confirm.
To access your Magnus Health account log into Magnus here and follow the directions. Use your Veracross portal username and password. If you have access issues click on “having trouble logging in” below the log in button and follow the prompts.
